How to Fill Out The Application
Important

Remember, Your trustee must be able to demonstrate to creditors that the asset values are reasonable, therefore, You must provide us with the following:

For Vehicles:

  • value of vehicles
  • proof of ownership
  • proof of insurance
  • your drivers license

For Real Estate:

  • proof of property value (purchase documents or letter from real estate agent or an appraisal)
  • your last mortgage statement or proof of mortgage value
Please answer every question. If a question does not apply to your situation, simply mark the box n/a. You must provide us with proper mailing addresses and account #'s for your creditors. This information can be obtained from your monthly statements or the telephone book.
What to do When Application Completed
After you have completed the application, please return it to our office by mail, email or fax it to us at (519) 633-4637. When we receive your application we will set up an appointment with you to sign the prescribed documents.
Filling Fees
When we file your bankruptcy or proposal, we are required to pay a filing fee effectively a tax payable to the Government). Therefore we cannot file your bankruptcy or proposal without a deposit.
Payments to the Trustee
Upon signing the bankruptcy or proposal documents, you will be required to provide (monthly) postdated cheques to the trustee to be applied towards the trustee fees.
